Where does the referral get sent?

Your Doctor/referring provider can fax the order to Cornerstone Physical Therapy & Aquatics in Sandpoint, ID or Granite Point Physical Therapy and Wellness in Athol, ID with the diagnosis codes appropriate to your condition. Based off of this referral our team will pair you with the balance/vestibular therapist from the start. The fax is 208.265.9192

Do you treat more than dizziness?
